Allied Health Building

Aims Community College Medical Assisting students and instructors will help provide exams alongside our partnering physicians. Appointments are $35 and will be available from 12 p.m. to 6 p.m. on Tuesday, July 21, in the Allied Health and Sciences Building on the Greeley Campus.

Two providers are seeing patients at the same time during the clinic, which means every 15-minute window has two openings.

REQUIRED — COMPLETE BEFORE YOUR APPOINTMENT

Please download, print and complete pages 1 and 2 only of the Colorado High School Activities Association (CHSAA) Preparticipation Physical Evaluation (PPE) form and bring the completed pages with you on July 21.

Pages 1 and 2 are the athlete's history and information sections, which the parent or guardian and athlete complete at home. Pages 3 and 4 are the physical exam form — the examining provider completes those at your appointment, so you do not need to fill them out in advance.

A parent or guardian must complete and sign pages 1 and 2 on behalf of any athlete under 18. The examining provider cannot complete the physical without these pages.
